Central Health Management, Inc. is seeking to hire a qualified Adult Nurse Practitioner to provide primary care for patients in the Virginia Beach/Hampton Roads area. This role will see patients in their home or Assisted Living facilities following their discharge and may see patients in a more acute setting on a weekend shift if desired.
In this role, you will assess the patient's condition and address the primary complaints, conduct a thorough review of medications, and provide treatment for medically necessary conditions. You may provide some minor wound care, and you will collaborate with our medical team to ensure each patient is provided with the best possible care, safety and comfort.
