Position Summary:

The Director of Care Management will lead the development and oversight of the Care Management and Resource Management programs, ensuring regulatory compliance and efficient patient care processes. This role collaborates closely with physicians, staff, and community partners to support patient-centered care and optimal outcomes.

Key Responsibilities Include:

• Leading a compliant and effective Care Management program.
• Supervising staff and ensuring role accountability.
• Conducting chart reviews for admissions, continued stays, and discharge planning.
• Coordinating discharge planning and social services functions.
• Acting as a liaison to community agencies for post-hospital support.
• Ensuring timely reporting of CPS/APS cases and educating staff on mandatory reporting.
• Participating in Patient Care Conferences and LOS meetings.

Qualifications:

• Bachelor’s Degree in Nursing (required); Master’s preferred.
• 4–6 years of clinical nursing experience.
• 3–5 years in care management or utilization review.
• 1–3 years of healthcare leadership experience.
• RN licensure and BCLS certification required.

• Strong leadership and team-building capabilities.
• Excellent communication and collaboration skills.
• Proficiency in EMR and care management software.
• Deep understanding of regulatory compliance and discharge planning.
