🔬 Now Hiring: Pharmacy Nurse Care Coordinator

📍 Location: Indianapolis, IN ( On-site with Local Travel)

💼 Position Type: Full-Time | Monday–Friday

🚗 Travel: 1–2 times per month (Client covers gas, lodging, meals)

🌟 What You’ll Do

As a Pharmacy Nurse Care Coordinator , you'll work closely with patients, caregivers, pharmacy staff, and medical providers to ensure seamless, personalized care. Your responsibilities will include:

• Instruct pharmacy patients on factor dosing, new product education, assess patient needs and direct patients to immediate care when necessary.
• Facilitate triage and prompt communication of nursing assessments to providers and ensure accurate electronic medical record (EMR) documentation.
• Effective cooperation and communication with other team members in the pharmacy and billing departments to ensure all patient needs are met and financial risks to the IHTC are minimized.
• Identify trends in patient care and needs and make recommendations/implement programs to improve patient services.
• Infusion log review and education.
• Home visits and needs assessments.
• Factor order completion and non-factor prescriptions.
• Home infusion training.

🔍 What You Bring

• Completion of an accredited nursing program
• A valid LPN or RN license in the state of Indiana
• At least 2 years of nursing experience , preferably in home health, infusion therapy, or a specialty care setting
• Strong communication and clinical assessment skills
• Ability to perform IV access procedures (ports, PICC lines, central line catheters, peripheral IVs)
• Confidence in educating patients and managing medication plans
• Comfort with occasional travel for home visits
• Ability to enable multi-factor authentication via smartphone or smart device (required for system access)
