Job Description

Equity Staffing Group is seeking a Registered Nurse (RN) to join our client, in Sebastian, FL in a temporary-to-hire role. The RN will provide chronic care management support to patients, coordinate care between providers, and assist with transitions of care. .

Job Functions

• Assessing health status and care needs
• Performing medication reconciliation
• Conducting motivational interviewing and goal setting
• Creating referrals to appropriate services and community resources
• Assisting with prescriptions, DME, transportation, and follow-up appointments

Qualifications

• Minimum 2 years in a clinical, hospital, or physician’s office setting
• Strong working knowledge of chronic conditions such as COPD/asthma, diabetes, CHF, and IHD
• Active, unrestricted Registered Nurse (RN) license in the state of Florida
• Current BLS certification
• Bachelor’s Degree in Nursing or Associate’s Degree in Nursing with 4+ years of related experience
